Implement the Tornado stream handler with range requests, format and
bitrate selection, on-the-fly transcoding, and cached transcode storage.
Add listening history endpoints, atomic play_count tracking, and the
TranscodedFile model. Harden database session management for tests and
add supporting test coverage.

Break the audio import, metadata extraction, and track list filter
